Integrated Platelet Pathophysiology
1966 - 1974
During this period, platelet biology matured from viewing platelets as simple clotting units to recognizing them as active modulators of hemostasis, thrombosis, and vascular pathology. The prominence of splenic pooling as a primary source of thrombocytopenia reframed diagnostic thinking around platelet sequestration rather than bone marrow production alone. The discovery that plasma contains platelet-derived particulate material rich in phospholipids with coagulant activity underscored a previously underappreciated extracellular facet of coagulation, shifting attention to the platelet's secretome. Methodological advances, including standardized bleeding time as a clinical screen and the use of pharmacologic triggers such as ristocetin to probe platelet aggregation, established functional testing as a cornerstone of hematology. In parallel, early work linking abnormal membrane glycoprotein patterns to defective aggregation foreshadowed later receptor biology and the emergence of receptor-targeted therapies. Overall, these themes reflect a shift toward integrated models of platelet function that connect cellular behavior, plasma-derived constituents, and receptor-mediated interactions.
